New patient forms

When you schedule your first therapy or assessment appointment, you will be asked to complete an intake packet providing basic information, insurance information (if you plan to file with insurance), consent, HIPPA privacy statement, and office policies. This packet is made available through the patient portal. You can complete these forms online from home, and submit them securely through the portal. The intake packet generally takes 30 minutes to finish, and should be completed prior to your appointment so as to not take any time away from your session. This will also allow the psychologist time to review the packet before your session.

Telehealth Services

Dr. Novinski provides the option of Telehealth sessions for individual psychotherapy. In-person/in-office sessions are preferred and recommended whenever possible. However, this is an option if you know in advance that you will be unable to get to the office (e.g., in case of inclement weather, recovery from illness/injury, COVID-19 precautions, etc.). Dr. Novinski recommends discussing the pros and cons of Telehealth in session so that you know whether this would be a good option for you.

Arriving for your appointment
